Extra Care

Extra care offers flexible care, with 24-hour support from social care and health teams. So, if you feel you or a family member meet the criteria for extra care support please contact us and we can arrange for you to be referred for a needs assessment by your Local Authority Social Services team. 

Following this, you will be advised if you meet the specific criteria for extra care and will be placed on a waiting list before being contacted when a suitable vacancy becomes available. 

Living in an extra care environment can help you maintain and improve your health and wellbeing and give you the opportunity to live in your own home whilst maintaining a level of independence that’s right for you. 

In addition, councils may commission a care and support provider who will provide an onsite support service available 24 hours a day/7 days a week basis to those who need it. 

Dementia specialist extra care

We can arrange for you to be referred for a needs assessment by your Local Authority Social Services team to see if you meet the specific criteria for specialist care. 

Schemes include:

  • Portland House, Newton-le-Willows – an award-winning dementia scheme offering flexible and comfortable care to support you to maintain an independent lifestyle. 
  • Carter House, St. Helens – a 15-apartment dementia scheme designed to give you the specialist care and support you need. 

Following this, we will advise if you meet the criteria for specialist care. If you are eligible, you will be placed on a waiting list and contacted as soon as a suitable vacancy becomes available. 

Specialist dementia housing offers flexible care for those who need it most, with 24-hour support on hand from experienced social care and health teams. 

The service provides flexible support packages including help with personal care, medication, preparing meals, cleaning and laundry. 

Salford

For Housing - Amblecoat Gardens

The scheme is situated on the corner of Amblecote Drive West and Clegg’s Lane, forming the gateway into the newly developed Amblecote neighbourhood. It is close to shops, banks, cafés, GP surgery, the local community church and much more. 

Amblecote Gardens comprises 66 stylish apartments designed for independent living, in the heart of the new Amblecote Green community.

For Housing - Bourke Gardens

In the heart of Walkden town centre, Bourke Gardens is a new extra care facility with 56 apartments (12 x two bed, 44 x one bed, two person) and has been designed using the principles of intentional communities – integrating the facility with the wider community so it can be used by the community and draw on the community for additional support. 

Especially designed for tenants who want an easy to manage and secure home with access to the care and support you need or may need in the future. 

Following an extra care assessment completed by the Salford Royal NHS Foundation Trust, you can enjoy: 

  • Security and peace of mind 24 hours a day, 365 days a year with a dedicated and experienced on site care team and state of the art security system.  
  • Flexible support packages include help with personal care, medication, preparing meals, cleaning and laundry.  
  • Regular social opportunities in the communal lounge area and delicious fresh and healthy meals prepared for you in the spacious cafeteria.  
  • Private, landscaped garden areas with seating, providing a tranquil place to relax and on-site car parking facilities

Monica Court is situated on Half Edge Lane, Eccles. Ellesmere Park is a pleasant, leafy residential area just a short distance from Eccles town centre and within proximity to Monton village. Eccles offers an array of shops, banks and cafes, GP surgeries, a railway station and much more.

Recently refurbished, Monica Court comprises of 47 one-bedroom apartments designed for independent living. With access to care and support 24 hours a day and regular housing management support, we also offer a wide range of events and activities helping to maintain and improve health and wellbeing.

The scheme is situated on Astley Road, Irlam, Salford, an established development. Access to the site is easy with local amenities a short distance away offering access to public transport, local shops, post office, library and GP surgeries.

Astley Court has recently been refurbished and comprises of 56 one-bedroom apartments designed for independent living. The scheme offers access to care and support 24 hours a day and regular housing management support. A range of events and activities will help to maintain and improve health and wellbeing.

Jackson Gardens

Jackson Gardens in Knowsley includes 64 apartments (5 x two bed, 59 x one bed) and 26 bungalows all for extra care. The scheme has been designed using the principles of intentional communities – integrating the facility with the wider community so it can be used by the community and draw on the community for additional support.

The scheme has been designed for customers who want an easy to manage and secure home with access to the care and support they need, or may need in the future.

Knowsley Metropolitan Borough Council decide who is entitled to live in Extra Care. The Extra Care team will complete a care needs assessment of individuals who want to apply.

Those who are eligible for Extra Care could receive the following support:

  • Security and peace of mind 24 hours a day, 365 days a year with a dedicated and experienced on site Care Team and state of the art security system.
  • Flexible support packages including help with personal care, medication, preparing meals, cleaning and laundry.
  • Regular social opportunities in the communal lounge area and delicious fresh and healthy meals prepared for you in the spacious cafeteria.
  • Landscaped garden areas with seating, providing a tranquil place to relax and on-site car parking facilities.

There are also 26 bungalows on the development, and these are available for affordable rent. Applicants will need to have a supported or low-level care need.

Please contact us if you feel you or a family member would benefit from additional support. A member of the Community & Wellbeing Team will contact you within 5 working days of receiving your enquiry and will complete an initial screening with you to support your application.

Your details will then be shared with Knowsley Metropolitan Borough Council who will carry out an adult social care needs assessment to see if you are eligible.
